How to order by FAX or Mail

To FAX or Mail an order, make selection(s) for purchase using our shopping cart and then proceed to CHECKOUT. Fill in your information in the Profile form fields and then Submit. Choose the FAX/Mail payment method and Generate Order Form. Confirm that your address information is correct before printing the order form. For FAX orders, please be sure your credit card information is included on the order form. For Mail orders, fill in your credit card information on the order form, or enclose a check or money order with your mailed order form.
